A growing number of states are stepping in to fix a severe shortage of home health aides by doing something that was once unthinkable in most Medicaid programs: paying parents to provide full-time nursing care for their own disabled or medically fragile children.

For years, federal rules largely blocked Medicaid from reimbursing a parent for care that went beyond what a typical family would provide. But with the national shortage of home nurses reaching crisis levels, many families say they have no choice but to quit their jobs to keep their kids alive. Now, states like California, New York, and Pennsylvania are quietly changing their policies, allowing parents to become paid caregivers under specific conditions.

The shift is not without controversy. Some lawmakers worry about fraud and whether parents might be incentivized to keep children in a medical state rather than pushing for recovery. But advocates argue that the current system is failing everyone. A child who needs a tracheostomy tube or ventilator support often cannot get a nurse to show up for a shift, leaving parents to perform complex medical tasks with no training and no pay.

Under the new models, parents must typically complete a certification process and document the hours they spend on skilled nursing tasks, such as administering IV medications or managing feeding tubes. The pay rates vary by state but generally fall below what a licensed nurse would earn, though for many families it is the difference between financial ruin and stability.

One mother in Ohio, whose 8-year-old son requires round-the-clock seizure monitoring, described the change as a lifeline. She had to leave her teaching job last year because no agency could staff her son's overnight care. Now, after completing a 40-hour training course, she gets paid for the eight hours a night she was already spending awake by his bedside.

Critics still question whether the long-term costs will balloon as more families apply. But supporters point out that the alternative is worse: children ending up in hospitals or institutions because no one can care for them at home. For now, the trend appears to be growing, with at least a dozen states exploring similar legislation. The message is clear: when the system cannot provide the worker, the parent becomes the professional.
